Register with the surgery
Join the surgery online in about ten minutes. You do not need proof of address, identification or immigration status — and you do not need to visit us.
Our practice area
We register patients who live in Willowbrook, Meadowvale, Loddon End and the surrounding villages — roughly everywhere inside the RG1 9 and RG2 8 postcode areas. If you are not sure, call us and reception will check your address straight away.

Look for your postcode above, or call us on 0118 496 0123 — checking takes a minute and saves you filling in a form we cannot accept.
Fill in the online registration form
The national Register with a GP surgery service walks you through it in about ten minutes. Answer as much as you can — anything missing can be added later.
We confirm your registration
Most registrations are confirmed within two working days. Your medical records transfer automatically from your previous practice — there is nothing to collect or deliver.

Good to know
No documents needed
You do not need proof of address, photo ID or immigration status to register. We register people experiencing homelessness — you can use the surgery's address. We are also a veteran-friendly practice.
Registering for someone else
You can register a child you are responsible for, or an adult you care for with their consent. Tell us you are a carer when you register — we will note it and connect you with local support.
Prefer paper?
Pick up a registration form at reception and hand it back in — no appointment needed. Free interpreters, including British Sign Language, can be arranged for any part of registration.
